Payment for Services

We accept the State Health Benefit Plan (except Kaiser Permanente), United Healthcare, Cigna, Medicaid and Medicare, and other plans. If your plan is not listed, please call us to confirm that we accept your insurance plan. Some of our service fees are based on a sliding scale. Unless arrangements have been made in advance, payments and any outstanding balances are expected at the time of service.

Self-Pay: If you do not have medical insurance, you will be considered a Self-Pay patient. Self-Pay patients must pay for the medical services received at the time of the visit. Sliding Scale can be applicable for some services based on your income.

We accept cash, checks (from financial institutions with a location in Georgia), credit cards, and debit cards (with a VISA or Mastercard logo).

Medical Records

Medical records are kept confidential. We will not release your medical information without a signed authorization from you.

Back to School (Hearing and Vision /Dental Screening)
  • Walk-in clinic for back-to-school immunization and health certificates.
  • To have information transferred to a Georgia 3300 form, parents need to bring a copy of a physical exam done in the last year, prior to school enrollment with hearing, vision, and dental screenings clearly marked.

Pregnancy Testing

No appointment is necessary. There is a $50 prepayment for services. If the pregnancy test is negative, there is a fee for service. Your visit will include:

  • Urine for a pregnancy test
  • Finger stick for lab
  • Medicaid application (one of the following documentation is needed: Birth Certificate, USA Passport or Naturalization Certificate, or I94/Green Card)
  • WIC referral
  • An appointment for Perinatal Case Management

Prenatal Case Management
  • To improve birth outcomes by assisting pregnant Medicaid-eligible women in gaining access to needed medical nutritional, social, educational other services.

STD
  • Walk-in clinic for STD evaluation/treatment and HIV testing
  • Fees are based on income using a sliding scale. You will be asked to pay a $50 prepayment before service. Fees will be discussed with you by the cashier.

Breast and Cervical Cancer Program

The Breast and Cervical Cancer Program was established to provide screening and follow-up services to low-income, uninsured/underinsured women throughout the state of Georgia because “Early Detection Saves Lives”.

Basic Eligibility for the Breast and Cervical Cancer Program:
  • Women 50-64 years of age
  • Women 40-49
  • With a family history of breast cancer (mother, sister, aunt) experiencing problems
  • DeKalb County Resident
  • Uninsured or Underinsured
  • Income less than 200% of poverty:
