Medical Reserve Corps Application

The following is provided in compliance with the Privacy Act of 1974:
The collection of information requested by the Medical Reserve Corps volunteer application is authorized by 5 U.S.C. 301 and Section 2 of Executive Order 13254 (issued January 29, 2002), which established the USA Freedom Corps. Providing the information is voluntary.

The primary purpose of the Medical Reserve Corps application is to allow individuals to indicate their interest in volunteering with the corps. Users may also elect to receive information about other DeKalb County Board of Health volunteer opportunities. All information submitted by an individual is available to the DeKalb County Board of Health’s Office of Emergency Preparedness and Office of Volunteer Services.

Public Disclosure

As a general rule, the DeKalb County Board of Health does not disclose any personally identifiable information collected online except where you have given us permission, or where the information is public information under the Georgia Open Records Act (O.C.G.A. §50-18-70 et seq.) or other applicable laws. Standard Orders

When you make an online purchase from the DeKalb County Board of Health, we request information from you on our order form. To purchase from us, you must provide contact information (like name and shipping address) and financial information (like credit card number and expiration date). We will require proof of identification of the person making a request before we can release a certified copy of a birth or death certificate.

Return Policy

In online purchases, only items that are identified on the site as eligible for return can be returned to the DeKalb County Board of Health for a refund.
